Be remunerated to conduct Medication Management Reviews
Medication Management Review (MMR) accreditation allows pharmacists to be remunerated for conducting Home Medicines Reviews (HMRs) and Residential Medication Management Reviews (RMMRs) through Community Pharmacy Agreement funding.
A medication management review is a thorough and collaborative assessment of a patient's medication regimen, conducted by an accredited pharmacist at the patient's place of residence.(I.e. a patient’s home for the case of a HMR or a residential aged care facility for the case of an RMMR). A referral from a GP or a specialist is required prior to the commencement of these service. For more information on the program rules see HMR Program Rules or RMMR Program Rules.
MMR aims to ensure the safe, effective, and appropriate use of medications by identifying and resolving any medicine-related issues that may hinder the patient's desired outcomes. It strives to enhance the patient's quality of life and health by employing a best practice approach, which involves collaboration between the GP, pharmacist, other relevant healthcare professionals, and, if necessary, the patient's caregiver. MMR also endeavors to enrich the patient's and healthcare professional's understanding and knowledge about the patient's medications, as well as foster a cooperative working relationship among healthcare team members for the betterment of the patient's health and wellbeing.

Stage 1 MMR Course
The College Stage 1 MMR course prepares you for the Stage 2 course, which ultimately leads to accreditation as a consultant pharmacist in Australia.

The stage 1 course is composed of online modules covering:
- interviewing the patient
- motivating patients for behaviour change
- reviewing medicines
- formulating a management plan
- communicating your plan to the referring doctor
- how to engage with doctors and other health professionals to build your MMR service.

Frequently Asked Questions
Has the Australian Association of Consultant Pharmacy (AACP) closed?+
Yes, as of 31st December 2022, AACP ceased operations. The AACP is no longer accepting applications to commence Stage 2 MMR accreditation.
I am a registered pharmacist. Do I need to become accredited to perform MMRs funded under the community pharmacy agreement such as HMRs and RMMRs?+
Yes, accreditation is required. Please refer to the Pharmacy Programs Administrator (PPA) for Home Medicines Review Program Rules and Residential Medication Management Reviews Program Rules.
What is the process for becoming an accredited pharmacist with the College?+
The College will accredit pharmacists after successful completion of all components of the College Medication Review course.
Can I complete the course online?+
All components of the College Medication Review course will be accessed online.
Do I need to reaccredit each year? +
It is our current understanding that accredited pharmacists no longer need to reaccredit to remain their MMR accreditation. The College continues to liaise with the PPA and the Department of Health on the maintenance of accreditation requirements.
In the meantime, the College recommends you continue to do Continuing Professional Development relevant to your scope of practice as an accredited pharmacist
Do I need to complete reaccreditation clinical MCQs every 3 years to retain accreditation?+
No. It is our current understanding that his is not the case.
How do I stay registered with the PPA to be remunerated for HMRs and RMMRs?+
The Department of Health and Aged Care have implemented a transition period whereby the AACP Medicine Review Numbers (MRN's) that were valid as of 31 December 2022, will remain valid until 30 June 2023 within the Pharmacy Programs Administrator (PPA).
How many CPD credits are accredited pharmacists required to complete each CPD year? +
Previously accredited with the AACP were required to complete 60 CPD credits each CPD year (with a maximum of 20 Group 1 CPD credits). The College will advise whether this requirement will change when more information becomes available.
Do I need to pay ongoing membership fees with a professional professional body that accredits pharmacists to remain accredited?+
The College is awaiting notification from the Department of Health and PPA regarding the ongoing accreditation requirements of existing accredited pharmacists.
I have partially completed my Stage 2 Medication Management Review course with the AACP. Can I complete my accreditation with the College? +
Yes, the College will offer you the ability to complete your course with no disadvantage. Please email us and we can sen you an enrolment form if would like to complete stage 2 with the College.
I have completed my Stage 1 course with another accredited provider. Can I complete my Stage 2 accreditation with the College?+
Yes, you will be able to obtain recognition of a Stage 1 Course and complete stage 2 with the College. Stage 1 Courses are valid for 3 years post completion.
Will the accreditation process remain the same? (i.e. Stage 1 and Stage 2)+
The current process of accreditation has not significantly change. However, each accredited provider will create their own pathway for accreditation with time.
My accreditation has lapsed/resigned for a period. Am I able to re-activate my accreditation with the College?+

What processes do professional bodies have to go through to provide a course that is eligible to accredit pharmacists?+
All accrediting bodies are required to provide a course (inclusive of training and assessment) which is aligned to a standard. The standard is currently being developed by the Australian Pharmacy Council and is likely to be released around July 2023.
